      This form satisfies the requirement for submitting a claim in writing, and if not used is sent to the claimant by the federal agency routinely. A "claim" is a written demand for the payment of a specified sum of money that is signed by the claimant or by a duly authorized agent or representative (unless the attorney as a special power of attorney for purpose of filing this claim, it is recommended that the claimant sign the form).
